Personal Loan Application Form 私人貸款申請表格

I/ We declare that all data and information stated in this application is true, accurate and complete. I/ We also declare that I/We have no intention to submit bankruptcy petition in the coming 12 months. I/We understand that I/We may contact Pan Asian Mortgage Company Limited (the “Company”) to obtain the address of the credit reference agency so as to access for a credit report if I/We shall consider it necessary. I/We further agree and authorize the Company to:
本人/吾等聲明本申請表內所載一切資料均屬真實、正確及完整。本人/吾等亦聲明本人/吾等並無意圖於未來12個月內申請破產。本人/吾等亦明白於有需要時可要求宏亞按揭證券有限公司("貴公司")給予有關信貸資料機構之地址以索取本人/吾等之信貸記錄報告。本人/吾等同意及授權貴公司可:-

1. Disclose, exchange all data and information contained herein together with particulars of my/our account(s) with the Company to any credit bureaus or agencies, banks, financial institutions, debt collection agencies or any other information sources for consideration of this application;
向任何信貸資料機構、銀行、財務機構、收數公司或其他資料來源透露、交換本申請表內一切有關本人/吾等之資料及本人/吾等於貴公司其他戶口之資料;

2. Access my/our credit data held by the credit reference agency and to obtain a credit report for consideration of this application and/or for further account review which include but not limited to my/our further loan application with the Company through telephone, Internet, fax or whatever channels acceptable to the Company. I/We understand and agree that the Company may conduct such reviews to determine if my/our existing credit/loan amount should be increased or decreased or remained unchanged;
接觸有關信貸資料機構查閱本人/吾等之信貸記錄及索取有關報告以作此項貸款申請及其後貴公司作戶口審查之用，包括但不限於本人/吾等其後向貴公司以電話、網上、傳真或其他貴公司認可之途徑再次申請貸款。本人/吾等明白及同意貴公司可進行該等戶口審查，以判斷本人/吾等之信貸額應否增加、減少或維持不變;

3. Approve or decline my/our application at its absolute discretion without stating any reasons and without returning to me/us the copies of supportive documents for this application;
有絕對權利批核或拒絕本人/吾等之貸款申請而毋需提出任何理由及毋需退回有關申請文件之副本予本人/吾等;

4. Collect all data that are required to support this application and I/We have been advised of my/our right to access, update or amend such data and information held by the Company or credit reference agency;
收集有關本人/吾等之資料以作本貸款申請之用及本人/吾等已獲貴公司通知本人/吾等可查閱及更正存於貴公司或信貸資料機構之個人資料;

5. Provide me from time to time with the marketing and product information from the Company through direct contact with me and/or access through any electronic means including but not limited to use of telephone calls, email, fax, SMS/MMS, pre-recorded voice/video messages; and
不時向本人∕吾等提供有關貴公司產品推廣之資料，包括但不限於可直接聯絡本人∕吾等及∕或透過電子訊息包括以電話、電郵、傳真、短訊∕多媒體短訊、預先 錄製語音 /視像訊息等方式將有關訊息傳送至本人∕吾等; 及

6. To deposit the loan proceeds to my/our designated bank account previously identified to you at time of application (if any) for loan disbursement.
根據本人/吾等於貸款申請時指定之銀行戶口（如適用者），將貸款額轉帳或存入該帳戶內。
